He collapsed again at home a few days later and was taken back to the hospital for more thorough testing, where he was diagnosed with exhaustion. Subsequently, the series' producers introduced a second team of detectives in 2005.

D'Onofrio appeared in the Homicide: Life on the Street sixth season episode "Subway" as John Lange. For this role he was nominated for an Emmy Award in the category Outstanding Guest Actor in a Drama Series.

He has also starred in many movies including Full Metal Jacket (with Matthew Modine, Adam Baldwin, and R. Lee Ermey), Adventures in Babysitting (with George Newbern, Anthony Rapp, Ron Canada, Bradley Whitford, Clark Johnson, and Lolita Davidovich), Mystic Pizza (with Julia Roberts, Annabeth Gish, Lili Taylor, Adam Stroke, Matt Damon, and Joanna Merlin), Men In Black (with Rip Torn and Siobhan Fallon), Household Saints (again with Lili Taylor), The Velocity of Gary as Valentino (with Olivia d'Abo), The Thirteenth Floor (with Craig Bierko), The Cell (with Jake Weber), Thumbsucker (with Lou Taylor Pucci), and Jurassic World (with Ty Simpkins and B.D. Wong).

D'Onofrio went on to star as Wilson Fisk/Kingpin (a character played in the 1994 Spider-Man animated series by Roscoe Lee Browne) on the Netflix action series Daredevil (2015–2018) with Charlie Cox as the title character, Jon Bernthal, who appeared on Law & Order: Criminal Intent episode Malignant as Frank Castle/The Punisher, Deborah Ann Woll as Karen Page, Elden Henson as Foggy Nelson, Ayelet Zurer as Vanessa Marianna Fisk, Peter McRobbie as Father Paul Lantom, Amy Rutberg as Marci Stahl, Geoffrey Cantor as Mitchell Ellison, Bob Gunton as Leland Owlsley, Michelle Hurd as DA Samantha Reyes, John Patrick Hayden as Jack Murdock (a character played by David Keith in the 2003 movie), and Vondie Curtis-Hall as Ben Urich. He reprises the role of Kingpin in the Disney+ series Hawkeye (with Vera Farmiga and Brian d'Arcy James), Echo, and Daredevil: Born Again with Jon Bernthal, Deborah Ann Woll, Elden Henson, Sandrine Holt as Vanessa Fisk, and Michael Gaston (coming in April 2025). He later starred on the TV series Godfather of Harlem (with Erik LaRay Harvey, Paul Sorvino, Luis Guzmán, Chazz Palminteri, and Giancarlo Esposito). He also appears in the films The Eyes of Tammy Faye (with Jessica Chastain) and The Unforgivable (with Viola Davis and Jon Bernthal). In 2021, he made guest appearances on the show Work in Progress in the episodes "Oh Say Can You See" and "FTP" as Robert Goren (though he is credited as himself) in fantasy sequences of the character Abby, who has a crush on the Goren character.[1][2]
